What toenail fungus is. And why it lingers.

Onychomycosis is a fungal infection of the nail. It is common, and it is more common with age.

What it looks like

A thick nail that turns yellow, white or brown, and may crumble, split or lift from the skin under it.

Why it happens

Fungi thrive in warm, damp places, such as sweaty shoes and shared showers. Athlete's foot, diabetes and poor circulation raise the risk.

Why it is hard to clear

The fungus grows as fine threads, called hyphae, in and under the hard nail, which shields it. Toenails grow slowly, so change takes months, and a big toenail can take longer than other nails.

Not every thick nail is fungus

An injury, psoriasis and other conditions can look the same. One of our physician assistant providers evaluates and diagnoses the nail before any treatment.

How the laser works. In plain words.

Light through the nail

The 1064 nm wavelength passes through the nail to the nail bed below, where the fungus lives.

Heat in the nail bed

Laser light turns into heat inside the nail. That heat is thought to damage the fungus it reaches in the nail and nail bed. The nail grows out clearer over 6 to 12 months.

Clear nail grows in

The laser does not change the damaged nail you see today. Any clearer nail grows in from the base, a little each month.

What the evidence says

This laser is FDA-cleared for the temporary increase of clear nail in onychomycosis. Studies of nail lasers report mixed results, and fungus can grow back. Prescription antifungal medication is the most studied treatment. We do not prescribe oral antifungal medication. You can discuss it with your primary care provider, podiatrist or dermatologist, and laser can be combined with it.

Before your first session.

One of our physician assistant providers evaluates and diagnoses the nail before any treatment, and tells you whether laser treatment is right for you. Tell us about diabetes, circulation or sensation problems before treatment. A nail that is painful, red, warm, swollen or draining needs care first. So does a dark brown or black streak or spot under a nail, because it can be something other than fungus.

What a visit is like.

  1. Consult

At your visit, one of our physician assistant providers evaluates and diagnoses the nail, asks about diabetes, circulation, sensation and medications, and tells you whether treatment is right for you.

  2. Prepare

Come with clean, bare nails, with no polish, so we can see and treat the whole nail.

  3. Treat

The laser passes over each affected nail and the skin around it. You feel warmth, and you walk out as usual.

  4. Repeat and watch

Most nails need several sessions, spaced weeks apart. Any clear nail shows as the nail grows, over months.

Candidacy and risks

Your health, your feet and the nail decide the plan.

Good candidates

Adults in good general health, once one of our physician assistant providers has diagnosed the nail as fungus.

Tell us first

Diabetes, circulation or sensation problems, a weakened immune system, medications, a recent nail injury, and your health history.

Sometimes no is safer

An open sore, an infected toe, or a nail that is painful, red, warm, swollen or draining is a reason to wait until it is treated and healed.

Common effects

Warmth during treatment and mild redness of the skin around the nail afterward, which usually settles quickly.

Less common risks

A brief hot pinch, and rarely a blister or burn of the nearby skin. Tell us at once if it feels too hot.

What it cannot do

It does not remove the damaged nail, it does not help every nail, and it does not stop fungus from growing again.

Pregnancy

Laser and light treatments are postponed during pregnancy. If you are pregnant, trying to become pregnant or breastfeeding, tell us before booking.

Aftercare and foot care

Dry feet and clean shoes lower the risk of fungus.

Keep your feet clean and dry, and change your socks every day. Wear breathable shoes, rotate pairs, and clean or replace old shoes. Wear sandals in shared showers, pools and gyms. Do not share nail clippers or files, and treat athlete's foot. Follow any medication plan from your primary care provider, podiatrist or dermatologist.

Call if something feels wrong

Call 941-216-5221 with any concern after treatment. Text 941-337-8461 with any other question. When we are closed, we still check texts, emails and voicemails, but not right away, so they are not an emergency line. For a true emergency, go to the nearest emergency room or call 911.

Nail fungus FAQ

Laser nail fungus treatment in Sarasota: your questions, answered.

Does laser treatment work for toenail fungus?

It can. Laser light turns into heat inside the nail. That heat is thought to damage the fungus it reaches in the nail and nail bed. The nail grows out clearer over 6 to 12 months. This laser is FDA-cleared for the temporary increase of clear nail in onychomycosis. Results vary: some nails grow out clearer, some change a little and some do not change.

How does the laser work on nail fungus?

The 1064 nm light passes through the nail. Laser light turns into heat inside the nail. That heat is thought to damage the fungus it reaches in the nail and nail bed. The nail grows out clearer over 6 to 12 months.

How many laser nail fungus sessions will I need?

Most nails need several sessions, spaced weeks apart. Your plan depends on how many nails are affected and how thick they are.

How long until I see a clearer nail?

Clearer nail grows in from the base, usually over 6 to 12 months. A big toenail can take longer to grow out fully.

Does laser nail fungus treatment hurt?

Most guests feel warmth, and sometimes a brief hot pinch. Tell us at once if it feels too hot, and we adjust or stop.

Is there downtime after laser nail fungus treatment?

Most guests walk out and go about their day. The skin around the nail may be a little red for a short time.

Is laser the same as nail fungus medication?

No. We do not prescribe oral antifungal medication. It is an option you can discuss with your primary care provider, podiatrist or dermatologist, and laser can be combined with it. Laser does not require the blood monitoring that oral antifungal medication can.

Can toenail fungus grow back after laser?

It can. The FDA clearance is for a temporary increase of clear nail, not a lasting result. Fungus does well in warm, damp shoes, so keeping feet dry, changing socks daily and wearing sandals in shared showers help lower the risk.

Who diagnoses the nail before treatment?

Antje Dangel, PA-C, or Jennifer Demers, PA-C, comes into the room at your visit to evaluate and diagnose the nail before any treatment, and tells you whether laser treatment is right for you. A nail that looks infected, or a dark streak under a nail, may need other care first.

Do I need to remove nail polish?
